MSHA cites procedure in mine accident - Daily Republican Register: Local News: VINCENNES, Ind. —A federal Mine Safety and Health Administration investigation into a May 15, 2012 mine construction injury at the Gibson South Mine near Owensville says the root cause of the accident was procedure that didn’t assure minimum risk exposure.

The report released Friday on MSHA’s website says James Palmer, a Frontier Kemper Constructors employee, was injured when a concrete hose knocked him to the floor of a work platform suspended in the shaft.

The incident broke Palmer’s left leg, and he died nine days later at his home in Marion, Ill., from complications of the injury.
