Columbia woman killed in work-related accident - Local / Metro - TheState.com: Lexington County authorities are investigating the death of a loading dock worker Wednesday night at the Otis Spunkmeyer plant on Overland Drive in West Columbia. Susan C. Henley, 47, of Columbia, was found pinned beneath a loading dock plate at some point after 11:28 p.m., the estimated time of the accident. She was pronounced dead on the scene.
